Steps to Apply:

2. Submit Required Documents:
   • Student’s birth certificate
   • Previous school report card (grades 1-5)
   • Standardized testing (NYS exams/MAPS)
   • Immunization records
   • Any relevant evaluations or educational plans (e.g., IEP, 504 Plan)
